Helen Chinn operates this site Mindful Sensory Occupational Therapy Services. This policy informs you how I use, store and protect your personal and sensitive information, in line with General Data Protection Regulations (GDPR) May 2018. By using this site you agree to the collection and use of information following this policy.
Website Data Collection
I ask for contact information, including your name and email address on my website in order to respond to your enquiry. My website does not use uses cookies to track your visit. I don’t use native social media ‘like’ or ‘sharing’ buttons that build your internet activity profiles.
Other Data Collection
To run my business, I need to collect personally identifiable information such as your name, address, and other contact details. I am also professionally required to keep clinical notes which may contain sensitive information. Such information is also necessary to fulfil the contracted service I will have been asked to provide (e.g. a diagnostic or other clinical service or report).
Data Sharing
The information you provide is confidential, subject to the exceptions laid out by my Terms and Conditions document which will be made available and signed in advance of commencing a contract. Exceptions include: when explicit consent is given to share information with another individual; information comes to light which indicates imminent harm to a child or vulnerable adult; sharing of unidentifiable information during professional supervision.
Data Storage
I am committed to ensuring that your information is stored securely. I have put in place suitable physical, electronic and organisational procedures to safeguard this. I use an online platform called Cliniko designed for healthcare practitioners which provides secure storage of client information. Your information will only be retained for as long as it can be reasonably assumed to be required and as according to professional guidelines (7 years from discharge).
Right to Access
You have a right to ask for a copy of your personal information, free of charge. You also have the right to ask me to amend or change any incorrect information I hold about you. Contact details can be found here.
Right to be Forgotten
Under GDPR, you have the right to ask an organisation to erase information held about you. Nevertheless, this is not an absolute right and applies only in certain circumstances. In practice, health professionals are unlikely to be able to fully comply with such a request because of the legal basis upon which we store and process data (to provide a contracted service, establish or defend against legal claims, and provide health or social care). However, please see above regarding your right to ask me to amend or change any incorrect information I hold about you.
Data Portability
As the client, you have the right to receive the personal information, which you previously provided, and to transfer that information to another party. For the purposes of the General Data Protection Regulations (GDPR) 2018, the “data controller” is Helen Chinn of Mindful Sensory Occupational Therapy Services.
